Flap Summary

Classification 

  • Muscle flap (Mathes and Nahai Type I) – One major 

Vascular Supply 

  • Arterial Supply = Sural artery 
  • Venous Drainage 

Innervation 

  • Motor = Tibial nerve 

Landmarks & Function 

  • Muscle Origin: Femoral epicondyles 
  • Muscle Insertion: Calcaneous via Achilles tendon
  • Action: Knee flexion, plantar flexion 

Variations 

  • Can be split into medial and lateral heads, each having its own neurovascular pedicle (medial head is longer) 

Applications 

  • Upper 1/3 leg coverage
  • Knee coverage

  • The gastrocnemius muscle has two muscular heads
    • Medial head
      • Origin: Posterior femur, superior to medial condyle, posterior to insertion of adductor magnus
    • lateral head
